Behind the ear tattoo: ideas, pain, healing and how it ages

Behind the ear tattoo guide: small design ideas, sizes that fit, how much it hurts, healing with hair and glasses, and pairing it with ear piercings.

A behind the ear tattoo sits on the small patch of skin over the bone just behind your earlobe, usually 1.5 to 4 cm tall. It's a favorite for tiny, delicate pieces: a little moon, a sprig of lavender, a musical note, a few stars trailing down. You can hide it with your hair down and show it off with a ponytail.

It's a great spot, but it has a few quirks. The area is small, the skin is thin over bone, and your hair, glasses and phone all touch it daily. Here's how to plan one well.

Behind the ear tattoo ideas

Designs that suit this spot are small, vertical or slightly curved, and simple. Popular choices include a crescent moon or tiny sun, a single flower stem following the curve of the ear, a few stars or dots trailing down toward the neck, a small heart or butterfly, a musical note, a short word in thin script, and a tiny wave or leaf.

Vertical shapes fit best because the space behind the ear is taller than it is wide. If you want something with more presence, a design can start behind the ear and curve down onto the side of the neck. That's a bigger step in visibility, so think about it carefully.

Size and placement

Most behind the ear tattoos are 1.5 to 4 cm. Your ear shape matters: ask your artist to place the stencil while your head is in a natural position, not tilted. Check it in two mirrors, from the side and from behind, because that's how other people will see it.

Keep in mind the back of the ear is easy to overlook in photos. If you want it to show more, place it slightly lower and further back, near the hairline. If you want it more hidden, tuck it closer to the ear itself. Also check which side you usually part your hair on. A tattoo on the side your hair falls over will be hidden most days, while one on the tucked side will show far more often than you might expect.

Does a behind the ear tattoo hurt?

Most people put it in the moderate range. The skin is thin and right over the mastoid bone, so you may feel a buzzing vibration through your skull, and the noise of the machine is very close. For many people that feels more strange than painful. Knowing the sound is coming, or listening to music in the other ear, makes it easier. Because these tattoos are tiny, the actual tattooing often takes 15 to 30 minutes.

Pain is personal and depends on your day, sleep and nerves. For a comparison with other spots, see tattoo pain by placement.

Healing behind the ear

This spot needs a bit of extra care because so much touches it. For the first two weeks, tie long hair back loosely so it doesn't stick to the tattoo, keep hair products like spray, oil and dry shampoo away from it, and avoid putting glasses arms or headphones directly on the area if you can. Sleep on the other side when possible.

Hold your phone to the opposite ear or use speaker mode, and wipe your phone screen often. Follow your artist's aftercare, and if you notice spreading redness, swelling or discharge, see a doctor.

Does it fade?

Behind the ear tattoos generally age fairly well because the area gets less sun than the wrist or hands. They can fade a little faster than an arm tattoo due to friction from hair, glasses and pillows. Thin lines may soften over the years, so keep enough space in the design and go slightly larger than the tiniest option. Fine line is the most common style here, and we cover its aging in minimalist fine line tattoos.

Pairing it with ear piercings

A behind the ear tattoo looks great with a curated ear. A small star tattoo next to a stacked lobe and helix creates a balanced, finished look, especially with your hair up. If you're planning both, think about the whole ear as one composition, and get ideas from our post on curated ear ideas.

Behind the ear or neck: which is right for you?

People often weigh behind the ear against the side or back of the neck. Behind the ear is smaller, more discreet and easier to hide, which makes it a gentle choice for a first tattoo or for jobs with strict dress codes. The neck gives you more room for a longer design, like a vertical line of script or a flower that runs down toward the shoulder, but it's visible with most tops.

The back of the neck, just under the hairline, is a third option. It's easy to cover with hair down or a collar, and it suits symmetrical designs around 3 to 6 cm. You can't see it yourself without two mirrors, though, so it's more for other people than for you.

If you're unsure, start behind the ear. A small piece there can later grow down onto the neck if you want more. Going the other direction is much harder, because a neck piece is hard to shrink or hide once it's done.

FAQ

Are behind the ear tattoos painful?

Most people find them moderate. The vibration on the bone and the loud buzzing are often more noticeable than the pain itself.

How long does a behind the ear tattoo take to heal?

Usually 2 to 3 weeks for the surface. Keep hair, hair products and glasses off it while it heals.

Can you hide a behind the ear tattoo?

Yes. With your hair down it's mostly hidden, which makes it a popular choice for people who need discreet tattoos.

How big can a behind the ear tattoo be?

Usually up to about 4 cm tall. Larger designs typically extend down onto the neck.
